The real reason this happened is fatigue. The Navy has been putting an emphasis on getting the ships out to sea more, but maintenance and training suffer as a result. When guys can't train, they can't qualify to stand certain watches meaning you have a shortage of officers and sailors who can stand watch. It's not unheard of to have an OOD on the midwatch who maybe has gotten 3 hours of sleep in the preceding 48 hours. That's a recipe for disaster.

She's the designated "special projects" boat so she does some things that fiction writers can't even dream of and we the public won't hear about until decades later.

I suggest reading "Blind Man's Bluff." When the Carter was built, they added a 100 foot section to her hull to carry on the type of missions that USS Parche took on in that book. (Parche had a similar 100 foot hull section added midway through her service life)
